Medical transport details
Pickup and drop-off locations, transport type needed, and any special equipment required for your safety.

Health insurance and billing
Insurance member ID, group number, and policyholder details to process claims correctly on your behalf.

Health information for care
Medical condition, diagnosis, medications, and allergies relevant to providing safe transport and proper care en route.
